SECTION ONE: APPLICANT INFORMATION. Worth up to 5 points.

• Does the applicant have clear contact information?
• Does the applicant have a well-defined mission statement that addresses a community need that fits within UWNCO's Results Statements:
  • All residents in north central Ohio will live in vibrant neighborhoods.
  • All children and adults in north central Ohio will be mentally and physically healthy.
  • All children in north central Ohio will achieve their full potential.
  • All residents in north central Ohio are able to meet their financial needs.

SECTION TWO: PROGRAM. Worth up to 10 points.

• Do the program activities directly relate to the results statement(s) chosen? Applicants were asked to choose results statement(s) from the following that best described the goal of their program:

SECTION THREE: TARGET POPULATIONS. Worth up to 25 points.

• Is there a clear plan for reaching the target population(s)?
  
• Does the program have a proactive plan for reaching a diverse population?
  
• Does the program proactively ensure equitable engagement?
  
• Is there a plan to engage the target population’s input in making program improvements?
  

• Does the applicant seem to understand how to create useful performance measures (PMs)?
• Do PMs emphasize answers to the question, "Is Anyone Better Off?"
• Do PMs directly relate to program goals? Will these PMs help them determine if they are reaching the program goal?
• Is there a clear plan for collection of PM data?

SECTION FIVE: TURN THE CURVE THINKING. Worth up to 25 points.

• Does the applicant show understanding about how to use PM data to improve their program?
• Does the applicant provide a thoughtful analysis of the PM chosen?
• Does the applicant’s choice of partners reflect consideration of all sides of the issue in question?
  
• Do the proposed activities directly relate back to the PM analysis? Are there clear indications these activities will improve the PM’s trend?
  
• Does the program have past success in achieving program targets?

SECTION SIX: FINANCIAL INFORMATION. Worth up to 10 points.

• Does the funding request seem reasonable for activities proposed and does NOT constitute entire program need?
  
• Is there a plan to supplement funding with in-kind and volunteer efforts when appropriate?
